Transportation:
The provision of transportation
for clients to and from nutrition sites/senior center,
medical/dental appointments, social service agencies, and
other essential services.
Congregate Dining Program: Daily provision of a hot,
nutritionally balanced meal in a socially stimulating
setting.
Care Management: The process of identification,
planning, coordination and linkage allowing the development
of a comprehensive plan of service.
Information and Referral: Provision of concrete
information to individuals concerning available community
services. This service also includes names, addresses,
phone numbers, and follow-up with the provider and/or
clients.
Outreach: The seeking out and identification of the
isolated hard to reach person(s) in order to assist them in
acquiring needed services and benefits.
Employment Referral Services: Working with the Title
V Senior Aides Program to assist older persons in locating
job opportunities.
Farmer's Market: In partnership with Clemson
Extension and the SC Department of Agriculture for the
distribution of vouchers to seniors for purchasing produce
grown by local farmers. (May -September)
Second Helpings: A food distribution project that
delivers to the senior centers grocery items that are
donated by local supermarkets and restaurants.
Energy Assistance Referral Program: An annual
agreement with BJ Economic Opportunity Commission which
qualifies older persons for financial assistance with their
winter heating bills.
Three Cheers for Volunteer Program: A three-year
volunteer program through the St. Helena Episcopal Church
comprised of seventy volunteers who assist with home
delivered meals in the Greater Beaufort area and St. Helena
Island. (Monday - Friday)
I-Care Insurance Counseling: A Department of Health &
Human Services Sponsored Program which assist older persons
with information regarding insurance claims and concerns.
Counseling is done by a regional group of volunteers.
Intergenerational Programs: An ongoing collaborative
effort with the Boys and Girls Club, elementary and high schools in affecting the quality
interactive programming between the seniors and youths of
the community.
Consumer Safety Information: Ongoing
information/presentation at the senior centers by the local
police, fire officials, and the SC Department of Consumer
Affairs in alerting seniors on various home, personal and
public safety precautions.
Home Care Services:
Home Delivered
Meals: Delivery of a
hot, nutritional balanced meal to the frail elderly who are
both unable to prepare it or unable to leave their homes to
attend a nutrition site.
Minor Home Repair
Referral: A program
identifying and referring seniors who reside in dilapidated
housing conditions to the appropriate agencies for
assistance and repair(s). |
